Refer Out Reasons

1. Transition Clinical Reject Reasons to Refer Out Reasons: When a consult enters the refer_out status, partners can now expect a consult reason. Consults that are referred out will still have a corresponding encounter and treatment plan in the Episode of care. The rejected status will exclusively be used for administrative reasons we cannot conduct care.

2. Addition of a consult status reasonSlug to the consult endpoint.
3. Partners need to transition to using the reasonSlug instead consult status reasons by August 1st 2024. The reasonSlug will stay consistent for workflow and reporting purposes however the reason string may be modified after this date.
