Urgent Care

To request an urgent care visit, patients must complete a clinical intake questionnaire (CIQ). This CIQ must include hard stop for red listed conditions that cannot be treated virtually. These intakes should support branching question logic, pharmacy selection, and image upload where applicable.

Urgent Care visits are typically offered through video. Condition specific intake forms for common conditions can be implemented to expand to async messaging / chat visits where State law allows.

The exact urgent care intake requirements will be defined by the clinical teams during implementation.
